If you are unhappy with your services or wish to report an issue, please call our Customer Service team at 833-409-4302 or complete this Complaint Form, and email it to info@cnshealthcare.org. Calls are answered between 8:30AM and 5:00PM. After hours, calls are answered by an answering service. 


You may also contact your county customer services/recipient rights center listed below.

Record Request

If you would like to speak with our Medical Records Department, please complete the form below. Someone from our Health Information Management Team will contact you with more information on how to obtain copies of your records. If you have a Case Manager, you can also reach out to them for assistance in completing the necessary forms to complete your request.



You can fax any request for records to the Health Information Management Team at 248-745-8250.
