If you are unhappy with your services or wish to report an issue, please call our Customer Service team at 833-409-4302 or complete this Complaint Form, and email it to info@cnshealthcare.org. Calls are answered between 8:30AM and 5:00PM. After hours, calls are answered by an answering service.

If you receive services in Macomb County and would like to file a grievance, it must be filed in writing or verbally to the Ombudsman. MCCMH must acknowledge your concern in writing and must work with you to resolve it within 90 days.

Records Request

If you would like to speak with our Medical Records Department, please complete the form below. Someone from our Health Information Management Team will contact you with more information on how to obtain copies of your records. If you have a Case Manager, you can also reach out to them for assistance in completing the necessary forms to complete your request.
